2002 – 2004 Motivational Interviewing Network of Trainers
Description. The Motivational Interviewing Network of Trainers (MINT) is an international collective of trainers in motivational interviewing and related methods (e.g., behavior change counseling, brief advice). The trainers come from diverse backgrounds, and their central interest is in improving the quality and effectiveness of counseling and consultations given to clients about behavior change. MINT members have completed a 3 or 4 day workshop designed to teach them the training methods, techniques and spirit of the MI approach. In order to encourage good practice standards, MINT membership is limited to trainers who have completed a training workshop for new MI trainers recognized by the MINT.
2002 – present Society for Research on Nicotine and Tobacco
Description. The Society for Research on Nicotine and Tobacco (SRNT) is the leading association focused on this special area of research. SRNT was founded in 1994 to coordinate and advance research on a broad array of topics ranging from the pharmacology of nicotine to the societal influences on use of tobacco.  The Society has flourished in the years since its inception and now has over 1100 members, in more than 40 countries around the world.

Description. The Kansas Public Health Association (KPHA) is the professional association for Kansas’s public health practitioners, professionals, and advocates.  Comprised of researchers/academia, health service providers, health advocates, local and state administrators, nurses, doctors, private or public organizations, safety net clinics, and health foundations, it is the oldest and largest Kansas public health organization created by Dr. Samuel Crumbine in 1920 to have a unique, multidisciplinary environment of professional exchange, study, and action, in public health practice and the public health policy process.

CV for Nicole L. Nollen

  1. TEACHING ACTIVITIES

My teaching interests lie in the areas of preventive medicine, social and behavioral aspects of health, and behavioral interventions, particularly the use of motivational interviewing (MI) counseling to facilitate health behavior change.  Since my arrival at KUMC, I have taught in the School of Medicine, the School of Allied Health, the School of Nursing, and the Master’s of Public Health (MPH) program. I direct two primary courses:  Social and Behavioral Aspects of Health (PRVM 818) and Motivational Interviewing in a Public Health Setting (PRVM 857).  The first course, Social and Behavioral Aspects of Health, is a required course, taken by all MPH students. The course, which I took over in 2006 and completely redesigned in 2009, provides an overview of social and behavioral aspects of public health including the relevance of psychological and social factors for health, the principles of health behavior change, the application of these principles in various health domains, and an introduction to health behavior and health promotion interventions. It begins with the rationale for studying social and behavioral aspects of health and examines select social and behavioral factors (e.g., gender, socioeconomic status, race/ethnicity) as they relate to physical well-being. It then focuses on well-established theories of health behavior and examines the role of psychological and social factors in specific health topics (e.g., obesity, cancer, cardiovascular disease, smoking).  For this course I organized the first-ever poverty simulation for the KUMC community in 2011. The simulation, put on by the Northeast Kansas Community Action Program, provided the opportunity for MPH students to experience first-hand the realities faced by low-income people. The simulation demonstrates my efforts to integrate novel and experiential-based approaches into my teaching methods. Students noted on their simulation evaluations that the experience was one of the most influential educational experiences they had encountered to-date.  The second course that I direct, Motivational Interviewing in a Public Health Setting, is an elective course in the MPH program and is cross-listed with the graduate programs in the Department of Dietetics and Nutrition.  I designed this course and have taught it once a year since 2005. This course, which introduces graduate students to Motivational Interviewing, its concepts, and to the subsequent skills required for helping people to change, has been extremely well-received.  Students have commented on their course evaluations that it is the most practical, useful, and applicable course taken during their graduate studies. My efforts in PRVM 818 and 857 led to Excellence in Public Health Teaching Awards in 2006 and 2010, demonstrating my effectiveness as a teacher.

 

I have also been very active teaching in the Medical School.  Since joining KUMC in 2005, I have regularly lectured to first and second year medical students in the following four courses: Health Promotion and Disease Prevention (ICM 801), Clinical Skills (ICM 850), Foundations in Medicine (CORE 800), and Gastrointestinal and Nutrition Block (CORE 820).  In addition, I lead small groups of medical students each year in issues related to the Critical Evaluation of Clinical Trials (CORE 800), Cancer Prevention and Screening (CORE 805), and Clinical Skills I/Practice Based Learning (ICM 849).  Finally, I regularly lecture to MPH students and the broader KUMC community through Clinical and Translational Research Seminar (PRVM 808) and Public Health Seminar (PRVM 805), to psychiatry residents at KUMC through Psychiatry Grand Rounds, to graduate nursing students at KUMC taking Adult/Gerontological Health Care II (NSRG 865), to graduate dietetics and nutrition students taking Health Behavior Counseling (DN 870) and have participated in leading a group of medical students during their Health of the Public (PRVM 960) course, dental hygiene students at UMKC taking Applied Nutrition (LSB 320), and clinical health psychology PhD students at UMKC taking Theoretical Foundations of Clinical Psychology (PSY 631) and Community Based Participatory Research (PSY 5580) .

 

I have also been active on MPH and PhD committees at KUMC and the University of Kansas.  Together, I have chaired 15 MPH Thesis/Practicum committees, 1 PhD Dissertation committee, and served on an additional 11 MPH Thesis/Practicum committees.  I have also advised 13 MPH students and 1 PhD student.

  1. SERVICE ACTIVITIES

I have been actively engaged in professional service activities outside of and within the medical center since coming to KUMC.  On a national level, I co-chair the Society of Behavioral Medicine’s Obesity and Eating Disorders Special Interest Group and, in this capacity, coordinate a yearly meeting of all members and network with national leaders in the obesity field.  As a result of my involvement with the Society of Behavioral Medicine’s Obesity and Eating Disorders Special Interest Group, I have been asked to serve on a 6 member Obesity and Diabetes Action Team that is charged with creating position statements and commentaries to inform ongoing federal obesity-related legislation. In addition, I have reviewed grants for the NIH, the Center for Disease Control and Prevention and for the Robert Wood Johnson Foundation, abstracts for the Society of Research on Nicotine and Tobacco, Society of Behavioral Medicine, and The Obesity Society, and manuscripts for 15 journals. At the state level, I have consulted on research projects conducted by the Kansas Department of Health and Environment and the Kansas Health Institute and have participated in drafting policy-level recommendations with the Healthy Kansas Early Detection and Screening Planning Committee of the Kansas Department of Health and Environment. At the community level, I have consulted with outreach staff at Swope Health Services on the provision of motivational interviewing counseling to clinic patients and provided training on community-based participatory research methods to members of the Latino and Native American communities in Kansas City. At KUMC, I am the Assistant Director of the K30 Distinguished Visiting Scholar Lecture Series, Chair the MPH Research Committee, am a member of the School of Medicine’s Election Committee and the Departmental Designee for all research involving human subjects, and have served on search committees for faculty and postdoctoral fellow recruitment.  I am also an active mentor for Taneisha Buchanan, Ph.D., a postdoctoral fellow at the University of Minnesota on a manuscript resulting from one of my recently funded research studies and an active mentor for Nia Thompson, MPH, and a second year medical student at KUMC on a manuscript resulting from my funded research.

  1. RESEARCH and SCHOLARLY ACTIVITIES:

    My research interests focus on three related areas: Smoking cessation among underserved populations, behavioral treatment of obesity among high risk groups, and systems factors in schools related to obesity. I am an investigator on 15 extramurally funded ongoing or completed grants and a principal investigator on 6 of these grants: A New Faculty in Public Health Award from Pfizer Global Pharmaceuticals ($130,000 direct costs), an NIH K12 ($533,520 direct costs), an Robert Wood Johnson Foundation Healthy Eating Research Award ($150,000 direct costs), a Sunflower Foundation, Health Care for Kansans award ($12,324), an Investigator-Initiated Research Contract with Pfizer Global Pharmaceuticals ($25,525 direct costs), a Kansas Masonic Cancer Research Institute Award ($35,000 direct costs), and a NIH K23 ($543,590). In addition, I have a pending R01 with NIH ($2,394,206) as the PI. This grant scored in the 19th percentile and received an impact score of 30 on the first submission and was resubmitted on July 5, 2011. In addition, I have two pending applications as a co-PI with the Robert Wood Johnson Foundation ($150,000) and as a co-investigator with NIH ($2,498,075). A brief description of research projects conducted in each of my three research areas and the significance of this work is included below.

 

In total, I have published 39 peer-reviewed journal articles (11 first-authored) and have an additional 5 articles (1 as senior author) under review. Of these 39 articles, 34 have been completed since I began my tenure as a postdoctoral fellow at KUMC in 2004. I routinely publish in high quality journals: 83% (23/36) of my manuscripts are published in journals ranked in the top two quartiles in their respective category (e.g., Psychology, Public, Environmental and Occupational Health, Medicine, Nutrition & Dietetics) (3 do not have available journal rankings).  In the last five years, 93% (26/28) of my manuscripts have been published in journals ranked in the top two quartiles. In total, my articles have been cited 660 times or an average of 19 citations per each published articles.  [NOTE: Journal rankings are summarized as opposed to impact factors because they provide a more accurate portrayal of the quality of my publications. For example, I have published two articles in Health Psychology, the top journal in my discipline, but the impact factor of this journal is relatively low (4.416) in comparison to all other science journals.]

 

Project summaries by focus area:

 

A.  Smoking Cessation among the Underserved.  African Americans (AA) smoke fewer cigarettes per day than the general population, but experience disproportionately greater smoking attributable morbidity and mortality. Disparities are due, in part, to limited evaluation of tobacco use treatment interventions for this high risk group. Our experienced, multi-disciplinary team has been working to improve tobacco use treatment outcomes in AA smokers for the last 10 years. The long-term goal of our research program is to reduce disparities in tobacco-related morbidity and mortality by evaluating promising pharmacotherapy and behavioral interventions for AA smokers. To this end, I have been involved with six studies: Health Behaviors of Smokers in Public Housing, NIH/NCI (Co-investigator, 2001-2005), a cluster-randomized R01 trial of smoking cessation and dietary change among public housing residents; Helping African American Light Smokers Quit, NIH/NCI (Co-investigator, 2001-2006), an NIH funded R01 clinical trial examining the efficacy of nicotine gum and motivational interviewing for smoking cessation among African American light smokers; Cigarette Craving in African Americans and Whites: An fMRI Study, Kansas City Area Life Sciences Institute (Co-investigator, 2003-2004), a pilot study examining potential racial/ethnic differences in nicotine craving among Black and White smokers using fMRI technology; Smoking Cessation Among the Homeless, American Lung Association (Co-investigator, 2003-2007) a randomized trial focusing on nicotine replacement and motivational interviewing counseling for smoking cessation among homeless individuals; Enhancing Smoking Cessation Treatment for African American Light Smokers, NIH/NCI (Co-investigator, 2006-2011), a double-blind, placebo-controlled, randomized R01 trial examining the efficacy of sustained release bupropion (active versus placebo) in combination with HE counseling for smoking cessation; and Enhancing Medication Compliance and Cessation Treatment Outcomes among African American Smokers, Kansas Masonic Cancer Research Institute and Pfizer Global Pharmaceuticals (Principal Investigator, 2008-2010), a randomized pilot study to estimate the effect of adherence support counseling in combination with Chantix for adherence and smoking cessation among AA smokers. In addition, I have an RO1 pending at NIH to better understand the disparities in quitting in African American and White smokers.  This proposal was reviewed in February, 2011 and received a impact score of 30 and a 19th percentile on the first submission. This proposal was resubmitted in July, 2011. These six studies have resulted in 23 published manuscripts and numerous presentations at national and international meetings. Our efforts in evaluating promising behavioral and pharmacotherapy treatments, psychosocial and biological mechanisms related to drug use and treatment outcomes, and identifying individuals for whom treatments are most effective are critical to advancing tobacco use treatment for AA smokers and attenuating tobacco-related health disparities.

B.  Behavioral Treatment of Obesity. Overweight and obesity in the US has reached epidemic proportions. Of particular concern is the rise within certain segments of the population, including children and adolescents, ethnic minorities, and the poor. Our team has been working to impact obesity in these high risk groups for the last 3 years.  The long-term goal of our research program is to reduce the incidence of overweight and obesity among children and adolescents, ethnic minorities, and the underserved through the development of novel and innovative treatment strategies. To this end, I have been involved in four studies: Bridging the Gap in Cancer Disparities Among Obese African American Women, American Cancer Society (Co-investigator, 2006-2009), a randomized trial examining whether the addition of motivational interviewing (MI) to a culturally-targeted behavioral weight loss program improves adherence to the intervention and increases weight loss behaviors among obese AA women; Enhancing Fruit and Vegetable Intake in Adolescent Girls through Handheld Computers, NIH/ORWH (Principal Investigator, 2006-2008), a randomized trial examining the feasibility and acceptability of a lifestyle modification program delivered to adolescents via handheld computer and its impact on weight, diet, and physical activity; Health Hawks Pediatric Obesity Program, Health Care Foundation of Greater Kansas City, (Co-investigator, 2007-2009), a two-year study examining if the addition of an intensive parent weight loss component to a comprehensive pediatric obesity program (Healthy Hawks Plus) improves weight loss of the target child compared to a standard pediatric obesity program (Healthy Hawks); and Using Handheld Computers to Improve Diet and Sedentary Behaviors among Adolescent Girls, NHLBI (Principal Investigator, 2009-2013), a career development award to complete mentoring and research activities related to obesity prevention among adolescent girls. To date, they have resulted in 10 published manuscript and numerous presentations at national and international meetings. More manuscripts will follow as work on these projects are completed. By examining novel approaches for preventing and treating obesity, this research area has the potential to alter the trajectory of obesity and reduce obesity-related morbidity and mortality among high risk population subgroups.

C.  System Factors in Schools Related to Obesity.  Mounting evidence points to the school environment as one contributing factor to childhood obesity. Schools have an important role to play in obesity prevention, but discrepant beliefs about the schools role and their financial dependence on foods sold through a la carte (ALC) and vending (i.e., competitive foods), makes changing the food environment difficult and unlikely, even among the most motivated schools. Our team has been working to better understand system factors in schools related to obesity for the last 4 years.  The long-term goal of our research program is to improve children’s diets and reduce the incidence of childhood obesity by decreasing the availability and improving the quality of competitive foods at school. To this end, I have been involved in three studies: The School Environment and Adolescent Obesity, Pfizer Global Pharmaceuticals, (Principal Investigator, 2004-2007), a two-part study comparing the availability and student purchasing of competitive foods in small and large high schools (Study 1) and examining school personnel’s perceptions of the school environment, its impact on obesity, and the potential impact of legislation regulating schools’ food/beverage offerings (Study 2); and The Financial Impact of A La Carte (ALC) Foods in Kansas Public Schools, Robert Wood Johnson Foundation (Principal Investigator, 2008-2010), a study examining how factors such as lower costs, higher meal prices, higher lunch participation, or use of supplementary funds are leveraged to effectively limit the availability of a la carte in Kansas schools. I obtained additional funding from The Sunflower Foundation, Health Care For Kansans to convene a meeting of key stakeholders to generate evidence-based recommendations aimed at stabilizing or modifying food service budgets while promoting healthier choices. These studies have resulted in 5 published manuscripts and numerous presentations at national and international meetings. In addition, I have two grant applications currently under review to continue this line of research.  The first, under review at The Robert Wood Johnson Foundation, will examine the impact of statewide vending policy changes on school meal programs in Kansas. The second, not funded by The Health Care Foundation of Greater Kansas City but under consideration with REACH Foundation, will bring healthier school meals to low income, urban children attending six elementary schools in Kansas City, Kansas (USD 500).  This innovative line of research examines adolescent obesity from a policy perspective and has the potential to directly impact state-level policies to reduce, eliminate, or improve the nutritional quality of ALC. In addition, this work combines efforts to address adolescent obesity from a behavioral and environmental/policy perspective. The combination of these two areas is critical to effectively slowing the trajectory of adolescent obesity.

Summary. This project was a supplement to The Financial Impact of a la carte Foods in Kansas Public Schools.  Funding allowed for the planning and convening of a group of key stakeholders. Results were presented to a from the Kansas State Department of Education, Kansas Department of Health and Environment, Coordinated School Health, child health advocacy organizations, health philanthropies, school food service and community organizations. Through a facilitated discussion, these stakeholders suggested policy and programmatic changes as next step opportunities to improve the health and financial stability of public school food environments at the local, district, state, and federal level. Recommendations were assembled into a research brief disseminated to local, district, state, and federal sources.

Summary. This is a five-year, randomized, controlled trial to evaluate the efficacy of extended bupropion treatment (6 months) versus standard bupropion treatment (7 weeks) to promote cessation and relapse prevention among African American light smokers. We will also evaluate medication adherence and nicotine intake in relation to smoking cessation outcomes. Role. For this project I will assist with training of study staff and provide weekly supervision of counseling. I will also assist with the dissemination of findings through manuscripts and conference submission.

  1. Other evidence of scholarship

March, 2010            Organized and facilitated a meeting of 50 local, state, and national stakeholders to discuss             findings from our study, Exploring the financial impact of a la carte offerings in Kansas public             schools. Through a facilitated discussion, stakeholders suggested policy and programmatic             changes as next step opportunities to improve the health and financial stability of public school             food environments at the local, district, state, and federal level. Recommendations were             assembled into a research brief disseminated to 1,500 local, district, state, and federal sources.
